Adult Clubs
Individual Club affiliation forms will be distributed at respective League AGMs. Clubs that are members of national and regional leagues will receive forms by post. The affiliation forms are pre-populated with data supplied from 2010/11 to minimise the time required to complete them. Please make sure you highlight any changes or errors and amend with the correct details.
The completed forms should be returned to the GFA Office by 30 June 2011 in order to avoid a late affiliation fine being applied.
Youth Clubs
Youth Club affiliation forms will mailed directly to the club secretary. Please ensure the Teams List is fully completed so that every team is recorded. e.g. U10 blues, reds, yellows etc. This form is also available in "Related Documents".

NOTE - The FA now requires every Youth League and Club to appoint a qualified Welfare Officer. Each Welfare Officer is required to have completed an FA Criminal Record Bureau (CRB) check and have attended The FA's Safeguarding Children Course and Welfare Officer Workshop. These requirements are now mandatory and failure to meet these will result in affiliation not being accepted. Click on the 'FA Guide to Welfare Officers' link to find out more.
New Clubs
New Clubs should download the appropriate blank affiliation form from "Related Documents" and once complete return it to the GFA Office. Please note that new clubs will need to contact adult and youth leagues in the county directly for respective membership and affiliation.

County Cup Entry
In order to avoid disappointment, it is recommended that club secretaries, particularly those responsible for youth clubs, check with the club’s coaches to ensure that your teams are entered into the county cups correctly. We cannot accept any late entries after the draws for the 1st Round of the different competitions have been made.
Public Liability Insurance
Public Liability Insurance cover is mandatory for all Clubs and organisations seeking affiliation to Gloucestershire FA. The Association offers amateur Clubs a policy through Bluefin Insurance Services Ltd., but it is not mandatory to purchase it. If you have your own Public Liability Insurance cover in place you must submit the original certificate and policy document confirming that cover is in place, together with your completed affiliation forms. Photocopies are not acceptable. The documents will be returned to you, together with your receipt, which will also record your affiliation number for the season.
